Safe & inclusive fieldwork card template
Before you depart for a field trip, consider printing KU SAIF cards for your research team. The card includes space for contact information for each individual's unit, unit leader and supervisor. It also includes contact information for reporting incidents to KU's Office of Civil Rights & Title IX, emergency contact information for the cardholder, basic emergency and CPR instructions, and a QR code that leads to the Safe + Inclusive Fieldwork portal on the KU Office of Research website.
Be sure to customize both sides of the card for each member of your research team before printing. You might choose to laminate the finished cards or insert them into plastic name badge holders to enhance durability in the field.
The KU SAIF card template files are designed to work with any 2-inch by 3.5-inch Avery business card template (e.g. 5371, 5871, 8371, 8871).

To download the SAIF card template, select the Word or InDesign option below. Be sure to click the download button in the upper left corner of your screen. Attempting to access the files by clicking on the yellow folders in the middle of your screen will not work.
Report an incident
Reports can be made to KU's Office of Civil Rights & Title IX in person, by phone or email, or via online complaint form by any individual, regardless of their affiliation with KU or the project.
